alternator pulley problem

Icon5 alternator pulley problem

i have a 89 honda prelude & i have recently replaced the alternator with a second hand one. electronically its working fine, but the pulley came loose while driving.
i had removed the alternator to have a look at it & the bolt holding the pulley on was completely loose.
i tightened the bolt back up & re-installed the alternator. i switched on the car & just after afew seconds the bolt came off again.
is there meant to be any washer in the pulley mechanism of the alternator?

if someone has any idea of what may be causing this i would appreciate to hear from you. thanks.
