I've posted around this question in other threads, but this problem is interesting. Like others, I'm getting two DTC's, P0171 (system too lean, bank 1) and P0170 (fuel trim, bank 1).

When I hook up my OBDII tool, and do a live data monitor, the Primary H02 sensor shows 0V. My live data recordings show a frame, at 1400RPMS or two where the Voltage jumps to 1.275V, and the STFT and LTFT go negative. The O2 V mostly sits at 0 throughout the sesssion. Intuitively, this says the O2 sensor needs replacing...or maybe a loose connection? When I previously ran the test, the O2 sensor would be sitting below 2V, but now it's pretty much registering nothing, except for the occasional spike.

My question is: If the O2 sensor goes bad, producing 0V, wouldn't the computer throw a code specifically for the O2 sensor? Would it only trigger the above codes without triggering the Primary Voltage H02 code?

I also have freeze frame data if anyone thinks that might be useful. I revved the engine to 3K-4K intermittently to see if things changed, but only noticed the STFT, LTFT and H02V changing.

Well, I replaced the O2 sensor, and now it's working -- so it's not an electrical issue. There's carbon-looking deposits on the O2 sensor and I noticed while under the car there's an oil leak somewhere...

What could cause the O2 sensor to foul up and fail? I know it's a new one that was replaced recently (last 6 months) by our Honda Service Tech.

Attached are photos. I've read elsewhere that this could be symptoms of a pin-hole leak in an exhaust passage of a cylinder head... Thoughts?

well if the O2 sensor is reading zero and your wire is fine then you have narrowed it down. see if its still under warranty

but an exhaust leak wont cause the sensor to go bad. but maybe throw a lean or rich code. i
